Quality improvement is a continuous service, which meets the expectations and satisfaction of patients, it is the collective quality which is applicable to all settings of health care including the pharmacy department which best defined as an attitude, an orientation that permeates an entire organization, and the way that an organization performs as internal and external business.

Purpose:

  • To meets the expectations and satisfaction of patients, hospital staff and managements in a cost effective manner.
  • To formulate an ongoing and effective quality improvement of the department.
  • To implement the guided principles under the realms of quality improvement management
  • To inspire all pharmacists to participate in the quality improvement programs in the department.
  • To inculcate the importance of quality improvement standards to the pharmacist the improve the quality and patients safety.

Policy:

It is the main trust and policy to focus and maintain the quality services to the patients by enhancing the quality improvement of pharmacy department.

To monitor the Pharmacy Quality. This can be achieved by using the following programs:

  • Quality improvement Program (QIP)
  • Quality Improvement Indicators (QII)
  • Quality Improvement Activities (QIA)

Procedures and responsibilities:

Pharmacy director & pharmacy supervisor plan different quality improvement activities within the pharmacy and nominating a quality improvement officer.

Quality improvement officer handles quality improvement programs, indicators, and activities in the quality improvement programs in the department.

The pharmacist is mandatory to cooperate with quality improvement officer and take part any quality improvement activities in the department.

To follow international model for quality improvement (PDCA): The Plan, Do, Check, Act methodology is utilized to plan, design, measure, assess and improve functions and processes related to medications safety & patient care

The following steps are how the pharmacy department may use the FPDCA methodology:

  • Find a process to improve
  • Organize a team / Utilization of multi-disciplinary team approach as needed.
  • Clarify the current State.
  • Understand root causes.
  • Select the Improvement, Plan the improvement, then Do the improvement.
  • Check the Results then act and determine next Steps.

Indicators will allow the pharmacy to identify problems, know their sizes, monitor progress and / or productivity and eventually ensure that quality improvement are sustained.

Data are collected according to the chosen indicators, report must be completed by the Q.I. Pharmacists and forwarded to Hospital QM Department on monthly and Quarterly basis.

The report must include at least two indicators;

Pharmacy department indicators will be updated annually according to error prone, and it includes but not limited to:

(Medication errors, ADR, emergency medications (STAT), not available medications, expired medications, high cost and high-volume medications)

  • Statistics for work load, waiting time, inventory and cost of some medications… etc.
  • Medication error reporting follow up.
  • Adverse Drug Reaction reporting follow up.
  • Restricted Antibiotics uses Analysis.
  • Teamwork and problem-solving techniques.
